Hand in Hand Developmental Support Plan

The Hand in Hand Developmental Support Plan is a template created by Infant Mental Health Promotion.  This tool is designed to be the next step after completing the ASQ-3 and ASQ:SE-2.  The Developmental Support Plan (DSP) is designed to help children reach the next developmental milestone by creating a clear cut plan for parents to follow.

The premise behind the Hand in Hand DSP is that attachment is the basis for healthy growth and development.  While the plan is designed to support the growth and development of the child, it focuses on the strategies that the family can implement in order to support this growth.

This document is designed to be prepared by professionals and then given to families.  The plan begins with a discussion of the importance of relationship building at this stage of the child's life.  The following 5 pages focus on goals for the child in each of the 5 domains covered by the ASQ-3 screening tool   The strategies are taken from a provided document, and inserted into the DSP.
